"The Correctional Investigator (CI) has exercised his right under section 193 of theCorrections and Conditional Release Act to table, in Parliament, a special report titled Fatal Response: An Investigation into the Preventable Death of Matthew Ryan Hines. The report examines the events that took place on May 26, 2015, leading up to, during and after the death in custody of Mr. Hines. In his report the CI concludes that Mr. Hines’ death was preventable and he provides 10 recommendations on the CSC’s policies and procedures, such as: the use of force (including the use of OC spray), as well as how the Service responds to medical emergencies and the sharing of medical information ... On behalf of CSC, I am accepting the recommendations put forward in the CI’s final report and the following response reflects CSC’s commitment to learn from Mr. Hines’ death and continually work to improve our response to individuals in medical distress"--Introduction, p. 1.
