The objectives of the investigation were to: gather all evidence related to the accident; conduct an analysis of the evidence; make findings as to the cause or factors contributing to it; make recommendations relating to the prevention of future similar accidents; and make any decision or order the Board can make, as appropriate, to prevent similar accidents from occurring.
