Preventing and managing chronic disease in First Nations communities : a guidance framework.: H34-313/1-2017E-PDF
"The Framework provides broad direction and identifies opportunities to improve the access of individuals, families, and communities to appropriate, culturally-relevant services and supports based on their needs at any point along the health continuum. It begins with promoting wellness, through to preventing disease, and then on through the various aspects of management and care, including palliative and end of life care. Beyond providing this support to communities in shaping their programs, the Framework can be used by the Government of Canada to supplement future programming decisions to ensure the best use of any available resources, building on the guidance of First Nations"--Purpose, p. 10.
