This booklet of Instructions and Definitions is to be used as a guide by Canadian hospitals in completing the Annual Return of Hospitals, Form HS-1 - Facilities and Services. A companion booklet is also available as a guide for the completion of the Annual Return of Hospitals, Form HS-2 - Financial Return”--Foreward.
