"The focus of the audit is the First Nations and Inuit Home and Community Care Program (the Program). In 1999, Health Canada launched a mandatory program that is a coordinated system of home and community-based health care services for First Nations and Inuit people of all ages with disabilities, chronic or acute illnesses and the elderly, to receive the care they need in their homes and communities. Health Canada provides funding through funding agreements for a continuum of basic essential services such as: client assessment and case management; home care nursing, personal care and home support, as well as in-home respite; and linkages, and referrals, as needed, to other health and social services. Care is delivered primarily by nurses, personal care workers and other community health and social development team members"--Executive summary, page i.
