"There has been surprisingly little analysis of the manner in which independent practitioners set their fees. This applies whether these charges are determined collectively or individually by the practitioners in question, and despite the growing significance of such activities. This study attempts to begin to fill this important gap in our understanding of income determination although, in the outcome, not as completely as one would wish. Basically, this is a survey of the procedures under which such fees are established. It does not purport to deal in any depth with the economic rationale, or with the equity or fairness of the resulting charges, let alone of the incomes to which they give rise"--Introduction, page 1.
